Before completing the application for SamUrgency Funding, you may want to read the following information about the procedures and what to expect once your application is sent.
If you want to go directly to the application, use one of these links:
You will be notified within 48 hours of receipt of your application, and again notified as soon as a decision has been made.
Upon receipt of the application, the board members of SamUrgency will discuss the application, and may request additional information as needed to come to a decision. The Veterinary Advisory Board of SamUrgency may also be contacted as necessary to assist Board Members in making the funding decision.
Before any funds will be sent, SamUrgency will require contact with the treating veterinarian, usually requesting information in writing regarding the diagnosis, treatment, and prognosis of the dog.
If the board of SamUrgency approves the application, we will contact our generous donators to see if they choose to sponsor this dog. Any donator choosing to sponsor this dog will send a check to the SamUrgency Treasurer, who will in turn send a payment to the treating veterinarian. In some cases an individual may be reimbursed for verified prior medical treatment expenditures (receipts required).
If the application is not approved, SamUrgency will also contact the applicant, providing an explanation of why it was not approved.
The applicant will be responsible for supervising final decisions on medical treatment, follow-up, fostering, and adoption. If this dog is not already neutered and will not be neutered during treatment, the veterinarian must explain why it is not medically feasible to neuter now and it is the applicant's responsibility to provide proof of neutering to SamUrgency as soon as medically possible.
The applicant must agree to stay in periodic contact with SamUrgency while treatment is ongoing and for a period of at least one year following completion of treatment.
Please understand that the SamUrgency board reviews all applications on a case-by-case basis, based largely on the prognosis of the dog. The majority of funding comes from Samoyed-owning donators who choose which dogs they wish to sponsor. Even if your application is approved by SamUrgency, that does not guarantee that there will be funding available to pay for all of the dog's treatment. Furthermore, SamUrgency is a funding source set up to help in emergency situations, and can not pay for normal rescue expenses, such as neutering, vaccinations, grooming, long-term boarding or transportations costs.
Please fill out the form below completely. You will receive a confirmation within 48 hours that we have received your application.
